A Brighter, More Connected Life: Collin’s Journey

Sometimes, the most meaningful changes in life are the ones that allow us to do something we might once have thought impossible.

Collin sits across a café table from his mum. They enjoy a coffee and spend time together. To someone passing by, it might seem like an ordinary afternoon.

But for Collin and his family, it represents how much has changed.

Collin has autism, a learning disability and Fragile X syndrome. He can experience difficulties with attention, impulsivity, restlessness and sensory processing, which can make everyday situations challenging.

When Collin r moved from Singapore to the UK with his family in 2021, he continued to live at home. By December 2022, supporting him at home had become increasingly difficult, and his family made the difficult decision for him to move into residential care. In 2024, Collin moved to one of Pathway’s supported living services.

Understanding Collin, beyond his challenges

The team took time to understand Collin’s routines, interests and preferences, recognising the importance of feeling comfortable, having choices and experiencing things that mattered to him.

By providing consistent, individualised support, offering choices and creating opportunities to explore his interests, the team helped Collin discover new experiences and enjoy more of life.

As one team member shares: “We’ve got to know Collin, what he enjoys and what matters to him. It’s lovely to see him trying new things, discovering new places and enjoying experiences that make him happy.”

One of Collin’s biggest dreams was to visit Madagascar and stay in a hotel. Rather than letting the distance or complexity stand in the way, the team explored how they could turn his wish into a safe, meaningful experience.

The result was an adventure created especially for him.

The team arranged an overnight hotel stay, complete with animal-themed surroundings and access to a small zoo, bringing the excitement of his dream into an experience he could enjoy.

Making it happen took careful planning. The team completed a full risk assessment, visited the venue beforehand, arranged additional staffing and connecting rooms, and shared Collin’s communication passport with the hotel. His parents were involved throughout, with reassurance at every stage.

Collin enjoyed a drink in the hotel restaurant, chatted with staff and embraced his night away. Afterwards, he proudly told his fellow tenants all about his adventure.

What began as a dream became an adventurous experience Collin will never forget.

A world with more possibilities

Collin now spends more time in the community, discovers new places and enjoys holidays and adventurous activities. His life has become richer and more varied, shaped increasingly by his interests and the experiences he can look forward to.

One of the most meaningful changes has been his relationship with his mum. Previously, spending time together could be difficult. Today, they can meet in the community, visit a café and enjoy time together.

It may seem like an ordinary afternoon, but for Collin and his family, it represents something significant: a part of family life that has become possible again.

The difference is in what becomes possible

Collin’s journey shows that meaningful progress is not always about dramatic changes. Sometimes, it is about making a dream possible, discovering a new place or spending time with someone you love.

For Pathway Healthcare, person-centred care means listening to what matters to each person, offering meaningful choices and finding ways to make new opportunities possible.

Care remains an important part of Collin’s life, but it is not the whole story.

The real difference is in everything that has become possible beyond it.
